Bring four quarts of water to a boil in a large pot and season liberally with salt. Add orecchiette and cook until al dente, about 10 minutes. Let the pasta be on the firmer side, so it can finish cooking in the sauce. Drain and set aside.

While pasta cooks, heat a large skillet or Dutch oven over medium high heat. Add sausage and break up into smaller pieces with a wooden spoon, sautéing until sausage is golden brown and no longer pink. Add the onions, garlic, and chili flake; sauté until onions are golden and translucent.

Add the broth and simmer over medium heat for 2 minutes. Add the cooked pasta, the broccoli rabe, and the lemon juice and zest. Simmer for 5 minutes until pasta has absorbed the sauce, and the broccoli rabe is tender. Season to taste.
